For all projects, you need an adult supervisor who is not a family member. The documents below are to assure that they understand the responsibilities and what will be asked of them.
Supervisor AGREEMENT
NOTE: The purpose of the agreement is for you to have a discussion with the potential supervisor about what CAS is and what your goals are for the project. Then ask if they will agree to be your supervisor. They do not have to be present all the time, but you will check in with them periodically throughout the project and at the end, they will need to complete the supervisor review. In most cases, this is not required for OIS Staff members who are supervising, but still not a bad idea. For supervisors outside of school, it is extremely important.

When the project is concluded, or at any time that your CAS Advisor requests it, you will use the supervisor review form to get the final reporet from the supervisor.

After the form(s) are signed, please scan–don´t photograph–them, label the file “Supervisor Agreement (or Review)” for [name of project], and upload it as a pdf in the Reflection / Evidence for the project.
